Bone augmentation and sinus lift are routine surgical procedures in oral surgery which solve the problem of lack of bone in the jaw during the implant placement process.

Why is augmentation necessary?

If the jawbone is not sufficient for the installation of dental implants, an augmentation, i.e. a bone regeneration (grafting) is necessary.

The loss of a tooth, whether it is a traumatic loss caused by an accident or the degeneration of a tooth that eventually needs to be extracted, results in retraction, which is the loss of jawbone at the site of the lost tooth.

Over time, retraction causes a noticeable lack of bone in this position. Therefore, if we want to successfully replace that tooth with a dental implant, it is necessary to increase the thickness and volume of the bone in that position. The process of bone formation is called augmentation. We can replace the bone with artificial biomaterial or by transplanting a bone block from the patient’s own bone mass.

Bone deficiency, the most common consequence of long-term missing teeth, is also the most common obstacle to the successful placement of implants. The speed of bone loss in an edentulous area is individual and depends on a number of factors such as diet or genetic predisposition. If there is insufficient bone for the installation of the implant, the procedure of augmentation (bone augmentation or bone regeneration) must apply.

Bone augmentation is a routine surgical procedure in which the necessary width and/or height of the jawbone is achieved by transplanting your own bone material and/or using artificial bone material. It is possible to perform the operation on both jaws if there is a minimal bone base of the patient for it. Depending on the size of the defect, the augmentation can be performed at the same time as the implant placement procedure.

In more severe cases, it is necessary to perform augmentation first and place the implants only after the period of time required for bone healing.

Methods of augmentation

Bone regeneration is possible using the patient’s own blood plasma according to the PRF protocol.

If there is no jawbone for placing implants, we can upgrade the bone in a completely natural and safe way using the PRF method and insert implants.

First we draw blood from the patient. After that, we separate the blood by processing it by coagulation with the help of a centrifuge in a special device. With this procedure we get a membrane for a safer, faster and easier result of implant placement in cases where the jawbone is missing.

The PRF bone replacement method is safe, completely harmless and contains no artificial materials apart from the patient’s blood. This is a very good basis for successful implant installation.

The autologous bone graft is still considered the highest standard in bone augmentation surgery because it has the strongest osteogenic potential. Another great advantage is that it is the patient’s own bone.

Autologous bone can be used in strips or in one piece as a bone block (bone graft). The location where the autologous bone is taken from can be intraoral (in the mouth) or extraoral (from elsewhere on the body). The quantity and quality of bone available for a bone graft is different for each donor site.

In order to ensure a sufficient amount of endogenous bone, we usually have to open the secondary donor surgical site in the mouth and surgically remove a sufficient amount of bone that is sufficient for the planned upgrade.

This procedure may sound and feel painful and uncomfortable, but all of our patients have had positive experiences with minimal discomfort and almost no postoperative pain or swelling. We perform the operation with a piezosurgical device that has an atraumatic effect on the tissue.

With this procedure, the bone is not ground down, but the desired part of the bone is removed using the principle of high-frequency vibrations. We use the Piezon Master Surgery® device from the Swiss company EMS, which is known for its long tradition and perfection in the manufacture of top medical instruments.

Piezosurgery is intended for use in oral surgery, implantology and periodontal surgery and ensures maximum surgical precision with a minimally invasive procedure.

These are materials of synthetic or animal origin. They are sufficient for bone formation, i.e. for the correction of small and medium-sized bone defects, and are usually available in granular form. In some cases, they should be combined with the body’s own bone to achieve better osteogenic potential, which then leads to faster bone formation.

FAQ about bone augmentation

Although you will likely feel normal again within a week or two, a dental bone graft can take from three to nine months to heal, sometimes even longer. Recovery time depends on several factors, including the type of graft, the area it was placed in, and your body’s ability to heal itself.

Most patients undergoing bone replacement procedures feel no pain and recover well when taking antibiotics. Your dentist will of course have to wait until the bone graft has grown together with the natural bone that is already in your mouth.

Bone grafting procedures typically take between 20 and 90 minutes to complete. This depends on the position of the graft, how much bone material is needed and whether other necessary dental procedures, such as a tooth extraction have to be performed.

The bone graft needs to heal for at least four months to be strong enough to support the dental implant. Ideally, the extraction takes place and we do the bone and membrane grafting at the same time. Then the patient has to recover for four months.
